Michigan’s no-fault insurance law provides important benefits after a car accident, no matter who caused the crash. These benefits can include coverage for medical bills, lost wages, attendant care, household services, medical mileage, and more. To receive certain benefits, especially wage loss, a properly completed disability certificate is often required.

This form allows your doctor to certify that your injuries prevent you from working and supports your claim for benefits under Michigan’s no-fault system. The form will indicate your inability to work or perform daily tasks, and will include specific restrictions, such as limitations on lifting, sitting, or walking.

Please bring this form to your doctor’s appointment. Your doctor will complete the form by indicating your medical restrictions and how long they expect those restrictions to last.

Once the form is completed, it allows the insurance company to review and process these benefits.

Your doctor must sign and date the form.

This documentation is required for the insurance company to approve and pay benefits. Without a disability certificate from your doctor, the insurance company will not issue payment.

After the form is completed, make a copy for your records and send the original to the insurance adjuster. The adjuster should provide this form to you, but if they do not, you can download it here.
